Non-Public Transportation Board of Education Request:
If your student needs transportation to a non-public school, you may use the form below and return the completed request form to the District for Board of Education approval.
Please download the pdf, complete the request form, and return the completed request form for processing.
For processing, you may:

Mail completed request form to Waterloo District Office, Attn: Superintendent’s Office, 109 Washington Street, Waterloo, NY 13165

Drop the completed request form off at the District Office (Monday—Friday, 7:30am—3:30pm)

Email the completed request form to the District Clerk, stephanie.wilson@waterloocsd.org
